Passport renewal application online: DS-5504 online.

Click on "Continue" under "Apply for a Passport Book" column and print the application. Be sure to sign and date your application. The DS-5504 form must be completed online.

Current valid passport

You are required to present your primary passport with your application. If your current passport is within 1 year of expiration you will need to renew your passport before applying. If you are applying for renewal of second passport, you need to submit your current and expired secondary passport with your application.

Letter of Authorization : Please print, sign, and date the letter and leave courier company name void.

Two (2) passport photos

Photos must be identical, taken within the past 6 months, 2x2 inches, and not from a vending machine. More on passport photo

Proof of the name change

A certified copy of either :

• A marriage certificate, or
• A name change court decree

Ups return label or FedEx (must be from sender address to same sender address)

1. Fill out Online DS-82 Application.
2. Current passport with two years validity.
3. Letter to the U.S. passport agency explaining the need of a second U.S. passport.
4. Evidence of Immediate travel (If expedite process is required). Following two copies are required:
   • Travel itinerary indicating reserved or confirmed.
   • Copy of round trip ticket indicating travel to a foreign country.
5. Two recent passport 2 x 2 inch photographs taken with in six months (Photograph quality requirements)
6. Government Passport Fees, following fees are applicable.
7. Authorization Letters (Download here) and (Download here) (that allows us to submit and receive your passport/application) Note: Please leave "Company Courier Name" blank.

Complete the following steps and provide Royal Visa with all necessary documents:

Make copies of the complete application to give to your Royal Visa agent before you seal the envelope at the post office

1. Fill the DS-11 Application form online.
   1. Do not sign this application form until you are instructed to during authorization.
   2. Important: When you’re done, click the Link that says create form.
   3. Once printed there should be Two bar codes on the page.
2. Fill the DS-64 Application form.
   1. This statement regarding lost or stolen passport and will be a part of your application.
3. Evidence of citizenship, at least one of the following must be provided.
   1. Previous U.S. passport (if it’s more than fifteen years old).
   2. Birth certificate.
   3. Certificate of citizenship or FS-240, DS-1350.
   4. Certificate of naturalization.
4. Evidence of individuality.
   1. Valid driver’s license with photograph, issued more than six months ago.
5. Evidence of urgent tour, provide two copies of one of the following.
   1. Facsimile of round trip travel document indicating travel to an overseas country.
   2. Traveling schedule stating reserved or confirmed.
   3. Business travelers could also submit a business letter of expedite, if the above is not available.
6. Passport photographs, taken by a professional.
   1. Provide two passport size photographs in color, front view and with a plain or light color background taken by a professional.
7. Authorization Letter (Download here), allows us to take steps as your agent.
   1. Print and complete two copies of the Authorization Letter.
   2. Please leave "Company Courier Name" blank.

Step 2: Authorization Processing

Bring all the required documents before appearing in front of an Acceptance Agent. This is what you will need.

1. Completed DS-11 Application form
2. Completed DS-64 application form
3. Evidence of Citizenship
4. Evidence of Individuality
5. Evidence of Urgent Tour
6. Authorization Letter

1. Parental authority: (Important, If the child is under the age of 16).
   1. Both parents must sign the application for passport.
   2. If only one parent can be present, you must have a signed and notarized Parental Consent Letter from the non-present parent. If sending in a Parental Consent Letter, the non-present parent must submit a clear copy of their driver’s license.
   3. If only one parent can be present, that parent must provide evidence that they have sole custody in order to apply for the child.

   1. Facsimile of round trip travel document indicating travel to an overseas country.
   2. Traveling schedule stating reserved or confirmed.
   3. Business travelers could also submit a Business Letter of Expedite, if the above is not available.
5. Passport photographs, taken by a professional.
   1. Provide two (2) passport size photographs (2"x 2") in color, front view and with a plain or light color background taken by a professional.
6. Authorization letter, allow us to take steps as your agent.
